Christa Pike's execution in Tennessee failed, leaving her unconscious and on a ventilator in a hospital. This was the second failed execution attempt in the state. Following the incident, the governor appointed a former U.S. Attorney to lead an independent review of how the Tennessee Department of Correction handled the execution.
